Qualifications:
- Must hold a valid and current Physical Therapist license in NYS.
- Experience with adults with I/DD is a plus.
- Strong documentation and communication skills are required.
What You’ll Do:
- Deliver evidence-based PT evaluations and treatments for adult patients with IDD in an outpatient setting.
- Create person-centered treatment plans; set goals that promote mobility, function, and independence.
- Document thoroughly and timely in the EMR; coordinate care with interdisciplinary providers and caregivers.
- Educate patients, families, and staff on home exercise programs, safety, and functional strategies.
- Contribute to a respectful, culturally aligned environment that supports patient dignity and inclusion.
